By accessing our website, booking an appointment, or using our diagnostic testing services, you agree to be bound by these Terms of Service. If you do not agree to these terms, please do not use our services. These terms may be updated from time to time, and continued use of our services constitutes acceptance of any changes.
2. Services Provided
One Stick Diagnostic Services LLC provides mobile and on-site diagnostic testing services including, but not limited to:
- Drug and alcohol testing (DOT and non-DOT)
- DNA and paternity testing
- Rapid diagnostic testing (COVID-19, etc.)
All tests are conducted by qualified healthcare professionals in accordance with applicable federal, state, and local regulations. We only offer CLIA-waived or appropriately certified testing services.
3. Scheduling and Appointments
Online Booking: Individual appointments can be scheduled through our Square Appointments booking system. Appointment times are subject to availability.
Group Bookings: For corporate testing, workplace screenings, or groups of five (5) or more individuals, please contact us directly by phone or email. Online booking is not available for group sessions.
Cancellations: Please provide at least 24 hours notice for cancellations or rescheduling. Same-day cancellations may be subject to a cancellation fee.
No-Shows: Failure to appear for a scheduled appointment without notice may result in a no-show fee and may require prepayment for future bookings.
4. Payment and Fees
Payment for services is due at the time of service unless prior arrangements have been made. We accept:
- Cash
- Credit/Debit cards (via Square)
- Check (for corporate accounts with pre-approval)
All prices are listed on our website and are subject to change with notice. Group and corporate rates are available upon request.
5. Identification Requirements
For all testing services, you must provide valid government-issued photo identification (driver's license, state ID, passport, or military ID). For DOT testing and legal DNA testing, additional identification verification may be required in accordance with federal regulations.
6. Test Results
Test results will be provided as follows:
- Rapid tests: Results provided at the time of testing or within the stated turnaround time.
- Lab-based tests: Results provided within the stated turnaround time via secure phone call, email, or patient portal as arranged at the time of testing.
- Chain-of-custody tests: Results follow strict documentation protocols as required by DOT or court standards.
We make every effort to deliver results within stated timeframes. Delays due to laboratory processing, shipping, or quality control measures are beyond our control.
7. Confidentiality and HIPAA
We are committed to protecting your health information in accordance with HIPAA regulations. All test results and personal health information are kept strictly confidential. 8. Limitations of Liability
One Stick Diagnostic Services LLC strives for accuracy in all testing services. However:
- We are not responsible for employment decisions, legal outcomes, or other consequences based on test results.
- Test results should be interpreted by qualified professionals and are not a substitute for medical diagnosis or treatment.
- We are not liable for delays caused by third-party laboratories, shipping carriers, or factors beyond our reasonable control.
9. Refund Policy
Refunds are provided on a case-by-case basis. Generally:
- No refunds for completed tests.
- If a test cannot be performed due to our error, a full refund or reschedule will be offered.
- Cancellation within 24 hours may qualify for partial credit toward future services.
10. Governing Law
These Terms of Service are governed by the laws of the Commonwealth of Pennsylvania. Any disputes arising from these terms shall be resolved in the courts of Cambria County, Pennsylvania.
